I have read and agree to the Video Game Club Parent Agreement & Liability Waiver. I understand this is a structured enrichment program, not childcare, taking place at a private residence in Montclair, NJ.

I release Video Game Club, Inc. and its staff from liability for minor injuries during normal program activities, except in cases of gross negligence. I authorize staff to administer basic first aid and contact emergency services if needed.

I understand the $155 session fee is non-refundable except in the case of a documented medical emergency. Video Game Club, Inc. may cancel sessions with adequate notice and will provide a credit or refund in that case.
